What is aromatase?
Aromatase is an enzyme found throughout the body that catalyzes the conversion of testosterone to estradiol, a form of estrogen, and is relevant to TRT patients because excessive aromatization can lead to elevated estrogen levels.
Aromatase is an enzyme present in body fat, muscle, bone, and other tissues that converts testosterone into estradiol (also called estrogen). During testosterone replacement therapy, aromatization occurs naturally as the body metabolizes some of the exogenous testosterone dose. The rate at which this happens varies significantly between individuals based on genetics, body composition, age, and overall health.
For men undergoing testosterone replacement therapy, understanding aromatase activity matters because excessive conversion can result in elevated estrogen levels, which may cause side effects such as water retention, gynecomastia (breast tissue growth), mood changes, and sexual dysfunction. Men with higher body fat percentages typically have more active aromatase in adipose tissue, leading to greater conversion of testosterone to estradiol at the same TRT dose.
Some TRT patients manage aromatase activity through several approaches: reducing excess body fat, adjusting testosterone dosing, modifying injection frequency, or using aromatase inhibitors (AI medications like anastrozole or letrozole) under medical supervision. However, complete aromatase suppression is not the goal, as some estrogen is necessary for bone health, cardiovascular function, and overall wellbeing. The balance depends on individual labs, symptoms, and clinical judgment from the treating provider.
