What is estradiol (E2) in men?
Estradiol (E2) is the active estrogen form produced when testosterone converts via the enzyme aromatase, and is monitored in men on TRT to maintain hormonal balance and manage side effects.
Estradiol, or E2, is the predominant form of estrogen present in male bodies. When men undergo testosterone replacement therapy, a portion of the exogenous testosterone they receive undergoes aromatization, an enzymatic conversion process where the enzyme aromatase transforms testosterone into estradiol. This is a natural metabolic pathway that occurs in adipose tissue, liver, and other organs.
During TRT, estradiol levels require monitoring because they directly influence treatment outcomes and side effects. When E2 becomes too high, men may experience gynecomastia (breast tissue growth), water retention, elevated blood pressure, and mood disturbances. Conversely, E2 levels that are too low can reduce bone density, diminish libido, and impair erectile function, since estrogen plays a protective role in male physiology despite being a traditionally female hormone.
