What is total testosterone?

Total testosterone is the blood test measurement that includes all testosterone in the bloodstream, both the hormone bound to proteins and the unbound portion, used to assess overall hormone levels.

Total testosterone is a blood test that measures all testosterone circulating in your body. The measurement includes testosterone bound to sex hormone-binding globulin (SHBG) and albumin, as well as the small fraction that remains unbound and biologically active. TRT providers use this test as the starting point for hormone assessment because it gives the full picture of how much testosterone your body contains overall.

Total testosterone differs from free testosterone in a critical way. Free testosterone represents only the unbound hormone available for immediate use by your cells, typically 1-3 percent of your total count. Many Miami-area TRT clinics order both tests together because a high total testosterone reading might actually indicate a problem if too much is bound and unavailable, while free testosterone tells the real functional story.

Reference ranges for total testosterone vary slightly between laboratories, but most clinics work with these benchmarks for adult males:
